SUMMARY CARE RECORD

NHS are introducing a Summary Care Record to improve the safety and quality of patient care across all services. This is an electronic record that can be viewed by healthcare staff, which will enable safer and more efficient access to essential information about you as a patient.

Summary Care Record contain only essential information such as your name, date of birth, address and NHS number, which will help to identify you. It will also list your current medication, allergies and any adverse reaction to medication you have had previously.

This electronic record will be automatically created for each patient by the practice, however if you do not wish to have this record please fill out the opt-out form and return it to the practice.
