HOW TO REGISTER
Welcome to Little St John's Surgery
If you would like to register with the practice you will need to be living within our practice area.
Please then visit the practice and ask the receptionist for a registration form to fill in and you will then be given further information. We ask all new patients to provide proof of identification. (e.g., passport, driving licence) and proof of address (e.g., utility bill or bank statement).
You will be offered a new patient health check with the practice nurse or the health care assistant.
Patients are registered with the practice rather than with individual doctors, however, a patient may request to be seen by a doctor of their choice.
The premises are readily accessible to the disabled, with the provision of a wheelchair ramp, handrails and disabled lavatory.

If you wish to register, click on the link below to open the form. When you have completed all of the details, click on the "Send" button, confirm your details, to send your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

WHEN YOU REGISTER, PLEASE ENSURE YOU FILL OUT THE MEDICAL QUESTIONNAIRE.
This is because it can take a considerable time for us to receive your medical records. There is an online version of this, below, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
It also allows us to have more information about you, for example, your mobile telephone number, which gives us more options to contact you later on.

Temporary Patient Registration
We provide a Temporary Patient service for anyone staying within our Practice area. This could be whilst on holiday, staying with relatives etc. Just call into the Surgery, fill in one of our Temporary Resident Forms and we will assist you.
