Summary Care records contain details of your key health information – Allergies, Adverse Reactions & Medications. These medical records can only be accessed by authorised staff in A&E departments,throughout England. This is to support better, safer prescribing of medications and up to date information on any allergies, drug sensitivity’s or adverse reactions for patients.
You will be asked permission before anyone is able to look at your Summary Care Record except in a case of emergency e.g. unconsciousness. If an authorised staff member has to do this they will make a permanent note on their computer system to state why they had to do so.
It is your personal choice if you wish to have a Summary Care Record. If you would like one you don’t need to do anything, it will happen automatically. If you DON’T want a Summary Care Record you will need to print off a SCR opt out form from our website: www.woodbridgedoctors.nhs.uk or ask at reception, fill in the form and hand into reception.
We require one form to be filled in for each family member. SCR_optout_form.
Little St John's Surgery confirm that SCR is automatically updated at least once a day to ensure your information is kept as up to date as possible.
GP to GP facility is the transfer of patient’s records between practices, when a patient registers or de-registers (permanent resident patients only).
Everyone should be registered with a Doctor, if you leave your GP and register with a new Practice, your medical records will be removed from your previous practice and forwarded on to the new Practice via NHS England, Paper records can take up to 2 weeks to be transferred to your new practice but with GP to GP transfer’s the record can be their a lot quicker making your care much safer.
Little St John's Surgery can confirm that GP to GP transfers are already active within this Practice and we send and receive patient records electronically where ever possible.
NHS England requires practices to offer the facility to enable practices online access to appointments, prescriptions and adverse reactions by 31st March 2015.
If you do not have a username and password to use this facility please come into the surgery with some identification and we will be happy to issue you with the information.
Information about you and the care you receive is shared, in a secure system, by healthcare staff to support your treatment and care.
It is important that we,the NHS, can use this information to plan and improve services for all patients. We would like to link information from all the different places where you receive care, such as your GP, hospital and community service, to help us provide a full picture. This will allow us to compare the care you received in one area against the care you received in another, so we can see what has worked best.
Information such as your postcode and NHS number, but not your name, will be used to link your records in a secure system, so your identity is protected. Information which does not reveal your identity can then be used by others, such as researchers and those planning health services, to make sure we provide the best care possible for everyone.
You have a choice. If you are happy for your information to be used in this way you do not have to do anything. If you have any concerns or wish to prevent this from happening, please speak to practice staff or download the opt out form below, complete it and return it to the practice
