PRIMARY CARE NETWORKS
We are a member of a Primary Care Network (PCN). This means we will be working closely with a number of Practices and health and care organisations to provide healthcare services to you.

During the course of our work we may share your information with these Practices and health care organisations/professionals. We will only share this information where it is necessary for healthcare purposes.
When we do this, we will always ensure that appropriate agreements are in place to protect your information and keep it safe and secure. This is also what the Law requires us to do.

First Contact Physiotherapists
The Peninsula Practice, Wickham Market Medical Centre, Little St Johns Surgery and Framfield House Surgery have been working together as part of the Deben Health Group (DHG) since 2012, and formed the DHG Primary Care Network (PCN) in 2019.
The PCN Board have created an on the day physiotherapy service to support patients across Deben Health Group and are pleased to announce that this service will launch Tuesday 1st September 2020.
Our Care Navigators can book a slot for patients, we recommend you contact your surgery in the normal way to use this physiotherapy service. In most cases it is more clinically appropriate to be triaged and have a consultation with a Physiotherapist, who is better qualified to treat musculoskeletal and joint problems, rather than see a GP or Nurse Practitioner and then be referred and wait longer for appropriate help.
This service has been set up in partnership with Allied Health Professionals (Suffolk), initially appointments will take place over the phone during a 20 minute slot between 9.30am until 5.30pm Monday – Friday, with face to face consultations happening only where clinically necessary at Framfield House Surgery in Woodbridge. Any patient registered with a Deben Health Group surgery can book an appointment.
Bethany Scott, MCSP, BSc (Hons), MSc and Kyle Tanner MCSP, BSc (Hons), MSc are experienced physiotherapists who will initially phone triage patients (due to COVID) and arrange face to face appointments where clinically necessary. They can we can request further investigations, following an initial triage assessment and in consultation with your own GP.
As this is a new service, we will be monitoring it carefully to check its progress and effectiveness over the coming months and your feedback would be welcomed to help us deliver the best provision to our DHG family.

PCN Pharmacy Team
The PCN Pharmacy team is led by Bhargavi Guvvala, Lead Clinical Pharmacist. She is supported by Mohammed Yasin, Clinical Pharmacist along with Pharmacy Technicians, Victoria Phipps and Kayleigh Gilmore.
PCN Patient Co-ordinators
The PCN Patient Co-ordinators work with patients in need of additional support across the Deben Health Group PCN practices. They act as a central point of contact, developing personalised care and support plans. The team work alongside our social prescriber, Dominique Dale as well as other care service providers and link workers.
The team co-ordinate our MDT meetings and they are also setting up Group Consultations. The scope of the role doesn’t end there and the PCN Board is reviewing what could be achieved by this team as they move forward.
The team are primary based at Rendlesham Medical Centre but also spend time in the practice as well working closely with our surgery team at Little St John’s Surgery. The combined skills and experience of each team member is of huge benefit to the PCN, we see the work of the Patient Co-ordinators as integral to meeting our PCN targets in the coming months and years.
